Chronic Disease Management Platform
Live platform helping patients manage and reverse Type 2 diabetes
The Challenge
An early-stage clinician-founder needed to turn a clinical concept into a live digital health product: mobile apps, CGM integration, behavioural science. No technical team, no architecture, no clear path to market.
What We Did
We owned the full product lifecycle: strategy, architecture, design, development, and launch. Built native iOS and Android apps with CGM integration, a gamification engine for patient engagement, and a clinician-facing EMR portal for care team oversight.
The Outcome
The platform shipped and is live. Supporting patients through structured care programs. Clinicians monitor patient journeys through the EMR portal while CGM data flows in real time to support clinical decision-making.

The Challenge
A US-based clinic needed remote monitoring for patients managing hypertension, diabetes, and kidney conditions. No existing digital infrastructure. Everything needed building from scratch.
What We Did
Built native iOS and Android applications with wearable device integration for continuous vitals tracking. Clinician dashboards for patient cohort oversight and structured monitoring workflows built for clinical reliability.
The Outcome
Over 5,000 patients actively use the platform. Continuous wearable data flows to clinician dashboards, enabling proactive care interventions and reducing the need for in-person visits.
AI-Powered Diagnostic Imaging
End-to-end SaMD compliance, from architecture to regulatory submission
The Challenge
An AI healthtech company had ML models for enriching diagnostic X-rays but needed clinical data infrastructure for DICOM image transfer and a regulatory pathway to bring it to market as Software as a Medical Device.
What We Did
Building the clinical data backbone: DICOM image transfer infrastructure connecting radiology labs, clinics, and hospitals with the AI platform. Delivering full SaMD regulatory documentation and structured testing frameworks for regulatory approval.
The Outcome
The platform is progressing through SaMD regulatory pathway with documentation, testing, and architecture aligned to regulatory expectations from the outset. Positioned for approval and commercial deployment.
Aged Care System Modernisation
National infrastructure modernisation roadmap delivered
The Challenge
A federal government agency separating from a larger department needed legacy systems assessed, data ownership established, and a modernisation path defined. Without disrupting care delivery to vulnerable Australians.
What We Did
Led the technology transition strategy: system selection, integration architecture, data ownership and migration strategy, and a phased modernisation roadmap across multiple stakeholder groups.
The Outcome
Delivered a comprehensive modernisation roadmap, from legacy assessment to target-state architecture, providing the foundation for improved technology infrastructure across Australia’s national aged care system.
